On February 5, 2001, a Notice of Violation was issued for a Severity Level III violation involving the failure of Weatherford International, Inc., a licensee of the State of Louisiana, to file NRC Form 241, " Report of Proposed Activities in Non-Agreement State," prior to conducting well logging operations using americum-241/beryllium sealed sources in offshore waters in areas of Federal jurisdiction.
